Webinar Series: Part 2
DATE: July 30, 2026
TIME: 10:00 AM to 11:00 AM (PT)
For HCCN Members Only
Raw attribution lists alone do not improve care; action does. In this second webinar of the series, participants will learn how to transform attribution data into clear, operational outreach priorities for patients who remain unseen. This session focuses on practical methods for identifying, stratifying, and engaging patients using evidence-based communication strategies.
Participants will review research-informed outreach approaches, explore pilot strategies using text, phone, and email, and learn how to integrate quality gaps, patient risk, and care management needs into actionable workflows. Real-world examples will demonstrate how health centers build effective outreach worklists that drive timely engagement. The webinar concludes with a structured “homework” assignment, enabling each health center to pilot outreach strategies and test new approaches before the next session.
